Evaluation Services
If you are unsure which service is appropriate, begin by completing our Evaluation Interview Form. The information you provide helps us understand your concerns and determine which services may be appropriate.
A screener is designed to identify potential areas of concern and determine whether further assessment or support may be beneficial. A comprehensive evaluation uses a broader assessment process to explore specific learning, cognitive, attention, developmental, or related concerns.
The process typically includes an initial information-gathering form, a parent or guardian interview, a review of relevant records when applicable, standardized assessment activities, and recommendations based on the information collected.
Testing time varies by service, age, and individual needs. Many appointments require approximately 1.5–3 hours. Longer evaluations may require additional sessions. You will receive information about the expected appointment length when your evaluation is scheduled.
When applicable, please provide recent report cards, standardized test scores, previous evaluation reports, IEP or 504 Plan documentation, teacher feedback, examples of schoolwork, and other records that may help us understand your child's needs.
The deliverables depend on the evaluation selected. Please confirm what is included in your specific service, including whether a written report, feedback session, and educational recommendations are provided.
